If you have been requested to be a Sponsor for either Baptism or Confirmation,  please make an appointment with Father in the parish center.

If you have been requested to be a Confirmation Sponsor here at Our Lady of the Valley Church, then you must contact the Faith Formation office for the appropriate form.
